you need a very high CMRR rate OPAmp for input buffer like OP-07/27/37--- is the most common one and an offset adjustment and gain control will be next after that if the speed is not importen try to make a v/f converter by using a cpu 16 bits counter alone with a op amp(as sample and hold), then you can have a very good system.
