I think the cheapest way would be not using components at all, just have a long shielded 16 wire cable with following wires, connect your 12 switches with 12 wires, and 3 for LEDs, 1 for common making a total of 16 wires. Debounce and filtering should be done at reciever end. Wires are real cheap after all and don't need even a pcb.
