This is the circuit , for my amusement and testing only..
I imagine it could be improved in any number of ways (use a rail to rail opamp of the 5vdc supply etc),but as it was a slight hack of a PCB I already had ,it sufficed)..
The resistors were all 1% tolerance..
I did add a CR low pass filter to smooth the output a little(not shown)..
There is a 2.5 v offset to the output ,but another opamp stage and a dc blocking capacitor will remove this(if you go to a balanced supply)..

the offset bias supply to the opamp (non inverting) is the same as the buffer..5vdc..
