I would bet dollars to donughts that the problem is a wiring error on your programmer circuit board. It most likely exists in the wiring between the microcontroller and the 40-pin socket. The error could be an open circuit, two connections shorted, or two connections cross connected. Your best bet is to very carefully ring out the wiring with an ohm meter and make entirely sure that every connection is correct.
