All you need to know about (E)ISA can be found in a book (name forgotten) published and sold by Annabooks, San Diego.
When I did (E)ISA interfaces I used various books and got royally screwed by all but the above. E.g. I had to redo a PCB because the mechanical dimensions shown for the ISA card was wrong in one of the books.
