I have one doubt.

If one of the phases has a low voltage which is outside the specified limit, then overall voltage across two phases will come down and I 'll be able to sense that one of the phases is down.But if two phases have low voltage , and if the combined voltage still results in DC voltage which is within the specified limit, there is no way of finding out. Is there any way out?

It is not important which phase is below/above the limit. We only need to know if any phase is below/above the limit.
