The Maxim part is actually a chipset (MAX250 - 251) but you have to add a transformer and optocouplers yourself. Is that what you mean by "haystack" Erik?

I myself have done it with discrete parts; four opto's, a DC-DC converter and any 'standard' RS-232 transceiver. Since my application communicates at 115k2, I've used fast opto's for Rx and Tx (HCPL-061A).
The circuits have been in use since 1997 and must have transmitted and received countless millions of bits without fail and, more to the point, without damage to my devices...
