1) Use separate grounds. 2) Quad and dual packages are availabale for opto-couplers. Consult the FARNEL catalog for a quick reference. 3) The most reliable power supply I have ever encountered is "PC power supply". It will well suit your voltage requirements as well. 4) Use ferrite beads in signal paths for extra safety. 5) EPABX is a complicated subject, I have designed one, if you wanna know further details, contact me.
