I have once seen something similar. Although I "knew" that the timer was started it turned out to be that the timer was not started under some very special conditions. After much frustration, I added a flag which was set when the timer was started (TR) and reset in the ISR and lo and behold when the TF was not set, neither was the "the timer was started" flag.
