good suggestion on the oscillator. assuming you're using a crystal (not a stand-alone oscillator) check the trace lengths and the loading caps. try removing the loading caps.

as for the errata mentioned, it ONLY applies when you've got the EA pin tied low, to execute from external code space only. this does not apply to your situation since you've got EA tied high.
