Yes I did ...but after the PCB was designed.
that is why I (and others) keep saying design do not 'test and fix'. You may have to start from scratch, the one recommendation I can give as a last recourse before redesign is: enclose the unit in a steel box and put ferrite beads around EVERY wire at the point where it enters the box.
