I have successfully been using the linear optocoupler IL300 from Siemens (now Infineon) in a medical application with similar requirements. This part should have the required isolation capability. The linearity is good enough for 10-12 bit resolution. It only needs 1 op-amp on the live side and one on the ground side plus some resistors.
