I have found that the problem was with the auto baud portion of the ISP. Since the equipment this board goes into is not ground referenced, I was using an optical isolation box for RS-232 communication. The opto couplers in the box were of the 1MBd variety, and that is too slow. The RB2 would see a stretched pulse and calculate the baud rate slightly too low. I redesigned the box with some 10MBd couplers, and now it works great.
