Erik, think of all the techniques you use to avoid defects. Your experience alone would probably mean that a major percentage of possible defects are avoided at the design stage. Unfortunately experience alone does not allow us to avoid all defects! I agree wholeheartedly with your comments regarding the labour cost!!
