Search the web for any 80's vintage synthesizer (schematics) and you will see how they did it back then. For instance Prophet-5, OBXa, Jupiter-8, etc.

This manual is HUGE, but it has a nice theory section.

In nutshell, the top of the pots are all connected to 5v, the bottom to ground and the wiper on each is connected to the A/D via a mux.
