1) You would normally buy a module intended for embedded use. Preferably a module not sold directly by any of the operators.

2) You normally don't. So if something is broken in the network, you have to buy one of their phones complete with a SIM card and then reproduce the problem just to get them to suddenly notice that their network is broken. But most probably you will have to fight through stupid people for a while unless you have the advantage to say that you have a large nomber of subscriptions and expect them to work. If the customer is a fortune-100 company, it can still be hard to get the dimwits to listen ;)

3) Approvals is everything - luckily I don't have to spend time with them. You want a module approved by the intended operator(s). Application and hw solution will determine what approvals you will need to get yourself for the product.

4) Then we are talking about a module intended for embedded use. They often have strange supply voltages. Some have built-in TCP/IP stack. Some have not. Think about number of bands you need, i.e. if you need to use the device outside US too. Vehicle use? Then think about the temperature span.
