I have found many defects due to poor design such as insufficient ESD protection of inputs, lack of decoupling capacitors, RC reset, "quick" programming schemes and missing overvoltage protection. I even suspect that some of the very few failures I have "blamed" om a semiconductor may have been due to the above.
