You have too complex a solution you are messing with. Toss the LM35 away!! Look at more modern parts and get yourself a TMP100 chip. This will measure temp from -55 to +130 C or some such with only the need for a single power supply. As far as the voltage measurement is concerned you could get yourself the MCP3221. Together this solution can connect to your monitoring controller with a single pair of SCL and SDA lines.
