A connector in the power system of one of the Gallileo satellites had a poorly designed connector between the system and the battery array. Though it had been specified by one of the JPL geniuses, it was not keyed in a way that prevented reverse power, so you know what eventually happened.
Fortunately, this was in time to allow for a detailed failure and effects analysis, and for appropriate repairs mitigating the risk of component failure to be undergone. It was a costly error. RE |
