There are some cases in which a supervisor might work, but a supervisor doesn't really address this problem. In some cases, it could destroy vital information from previous events.

While a one-shot, because of its absolute imprecision, is certainly justification for firing a designer, there are occasions where they will solve a problem such as this one. Other hardware would probably do it too, and not get you fired.
