Phtml files can be created with any standard text editor, such as Notepad (Windows) or vi (Unix) or anything else that saves in pure ASCII format. This can also be programs such as FrontPage, although FrontPage tends to add a lot of other unnecessary junk to the HTML files.
For the purposes of everyone here, a phtml file is just an html file that has been renamed to phtml. Create your "html" file, test it using your browser locally on your machine before uploading it, and then rename it to phtml before uploading it. That's all you should need to do.
