I think my rate would be around 1:200 or so - I seldom do these things. Bu then I am using a good fast short tester with sharp probes a lot...
One of our techies repaired around 100 boards (mostly TQFP160 replacement, sometimes TQFP100, and the worst, PLCC44) with no special tools and no one failed pin. Similarly good is one of our HW developers, building his prototypes; except he sometimes turns TQFPs around, it's loads of fun seing (and hearing) him finding and repairing that...

So I think the 100ppm and less is a quite realistic rate for the skilled.
