A four layer board is made as a lamination of two thin circuit boards that are each 2-sided. One of these two thin circuit boards can have copper only on one side with the result being a laminated three layer board. (Note that in all likelyhood the manufacturer would probably start with the thin boards each being 2-sided and then completely etch the copper off one side of one of them!).
