when i'm using keil to create a new project, it asks me if i want a standard 8051 startup code to be generated.

I think what it actually asks is whether you want the standard startup file added to your project.

Is that something i should be concerned with?

The standard startup file will be used anyway if you don't add it to your project - so initially it makes no difference.

At a later stage, you may well find that you need to modify the startup file according to your needs. At this point, it's a little more convenient if the file is already there in your Project.

Note that there is a whole section about startup.a51 in the Manual
