That proves that the assembly is faulty - it does not tell you whether it's a component or a manufacturing fault.

Are the wires put on by the original connector manufacturer, or are you using an assembly company that buys-in connectors and wire and sells-on the resulting assemblies?
If it's the latter, you need to work with your supplier to find out if they have a quality problem, or if it's with their supplier...
