I don't have any information about tooling used since I purchased crimped ready made connectors. Are the wires put on by the original connector manufacturer, or are you using an assembly company that buys-in connectors and wire and sells-on the resulting assemblies? If it's the latter, you need to work with your supplier to find out if they have a quality problem, or if it's with their supplier...
