I think (again this is only what I think, Mehdi can correct me) that the GND symbol shown on that circuit diagram is not for the local earth, but for a negative pole of a battery (isolated supply of the tester).
So it is NOT measuring the difference of the lines to local earth, but is supposed to measure the difference in voltages between tip and ring. Well, I am not convinced this will work satisfactorily enough (e.g. that the "dim LED" is enough information to decide whether it is a exchanged wires situation).
