Now imagine what happens when PSEN sources high level but is connected to ground externally. It is overloaded and may be damaged.

I assume this is discussing Atmel
1) if you use NoTouch you can forget about PSEN low. (search here, someone published a version for Atmel).
2) the Philips recommendeation is to put a cpacitor in series with a switch to ground that takes care of the overload.
