I gave up on the ISP-to-ICP bridge method of programming parts. We never got better than about a 25% success rate, even after modifying the power supply circuit (NOTE that there are two versions of the AN10258 app note and also multiple versions of the bridge firmware). Also we ended searching for an ICP programmer and turned up nothing HOWEVER the Acqura people have provided a way to use their PDS900 programmer for just this purpose. We were using it as an ICE anyway, so all we needed was to fashion a cable. See this thread for more:
