For Top, Bottom, Solder Mask and Component print layers:

* RS274-X - any aperture sizes acceptable.

  boards without holes (not preffered will bear $10 file

* PS,PRN - Postscript interpreted format, good only for single

* DXF - Autocad format, good only for single side boards without

* BMP,TIFF,GIF,PCX - pucture format, good only for single side

This Czech company http://www.printed.cz offers also "nonstandard inputs", including copy of an existing PCB, printed layout, hand drawn layout etc. (unfortunately, this text I found only in Czech), including the reconstruction of the drill.

There must be many similar around.
