1. C2 and SW5 must be connected to pin 20 of IC1!

2. Connect an additional 100nF ceramic (with a 10µF tantal in parallel) DIRECTLY between pin 20 and pin 10 of IC1, with very shortest connections!!!!!!!!!!!! Every millimeter counts! You don't have any working supply voltage decoupling for IC1 up to now, because the supercap C1 is way too far away from IC1 and by this isn't able to decouple high frequencies!!
