Probably for small runs for prototyping, maybe 2 or 3 (instead of spending time building it by hand). Then sometimes for medium-size runs (20-50) if I come up with something I actually want to produce. The setup charges is what makes the existing services relatively expensive for those of us that are working independently or are experimenting. Erik is right that those with big budgets already have their contacts and that this is a small cost of their business, but I do think there's a segment that could benefit from such a service if there's any way to reduce the setup cost. I'd definitely consider myself part of that segment. ExpressPCB has a nice service for their MiniBoards. No silkscreen, $51 for three boards, and have next-day shipping.
