For mass-market providers (eg, Microsith), it would surely be impractical for them to offer individual service & maintainability to evey single customer?
Just the same as mass-market car manufacturers cannot offer a car precisely tailored to each individual customers specific needs.

But surely a bespoke software company should be able to manage this?

At a cost. of couse...
