You need to stop being general.
you say 0-5 V output now you say relay. an AND gate will not drive a relay unless it is tiny. You need a driver. What Relay how big? how many amp is the load? You may need a 12 volt relay.

First voltage now Temperature sensor what kind? thermistor? Thermocouple? IC (linear , I2C, One-wire)?
Bits 3 bits is not 3 digits. 8 bits = 256 5.0 volts / 256 = 0.01953125V per bit.

What are you trying to do? You have told us your plan, but it is based on many misconceptions.

Does any one in your Group Know C, ASM or BASIC?

You can not get anyone to just code this at this point. After several days you still do not have a clear specification. It does not appear that anyone can code it without real hardware.

Is this for 1 piece or many?

Goggle "BASIC Stamp" it is a cheap module with BASIC built in. You choice of CPU is poor. You can get a single chip that has all you need and has cheap tools available. There are many pre made boards available that may fit you needs, relays included.
