Do you mean that buffers or something like that should be used before the ports leave the board?

Yes, always

1) the "weak pull-up" for a '1' on a port pin is way too high impedance for any kind of noise resistance when used as outputs

2) when used as inputs, standatd CMOS can handle 20mA, the typical uC can handle a mA or so.
