You are right there. I should have caught at least those two, as well as in all reality the incorrect polarization of the caps, even the wrong pin connections to the DB9 where obvious mistakes that I would have caught if I had looked a little closer and compare a little more with the example schematics I was using.

Lesson learned...next time at 8:30am sleep first then review and post schematic =)
