Just a quick look shows that the supervisor is hooked up wrong. There is no reason to have a capacitor in series with the ground and Vcc of this chip. Remember capacitors are open circuits to DC!

Also, once again I would suggest moving your decoupling capacitors to be next to their respective IC's.
